If you are a man somewhere between 40 and 70, you have almost certainly been told that intermittent fasting will fix your waistline, your energy, your blood sugar, and possibly your marriage. The internet is loud about this. The research is quieter, and a lot more careful.
Here is the honest summary before we go any further: intermittent fasting is a legitimate, reasonably well-studied eating pattern that helps some people lose weight and improve some markers of metabolic health. It has not been shown to be clearly better than simply eating fewer calories in a way you can sustain. It is not appropriate for everyone. And several of the most popular claims made about it — that it “resets your hormones” or gives your organs a “rest” — are not established science.
This article walks through what the human evidence supports, what it does not, and what a thoughtful 45- or 60-year-old man might reasonably do with that information.
What “Intermittent Fasting” Actually Means
The term covers several different patterns, and they are not interchangeable in the research.
Time-restricted eating (TRE)
Eating within a set window each day — commonly 8, 10, or 12 hours — and consuming only water or non-caloric drinks the rest of the time. “16:8” is the best-known version. This is the mildest and most-studied form.
Alternate-day fasting (ADF)
Alternating between normal eating days and days with either no food or a very small amount (often around 500 calories).
The 5:2 approach
Eating normally five days a week and sharply restricting calories on two non-consecutive days.
Longer fasts
Anything beyond roughly 24 hours. These are far less studied in healthy free-living adults, carry more risk, and are outside the scope of what most people should try on their own.
When someone says “the research on fasting,” ask which of these they mean. A 12-hour overnight window and a 36-hour water fast are not the same intervention.
What the Human Evidence Reasonably Supports
Modest weight loss, mostly through eating less. Multiple randomized trials and systematic reviews find that intermittent fasting produces weight loss in the range of a few percent of body weight over several months. The most instructive finding is why: when trials directly compare intermittent fasting against daily calorie restriction matched for total calories, the two usually perform about the same. A well-known randomized trial by Lowe and colleagues (2020) found that 16:8 time-restricted eating produced only small weight loss and no significant advantage over three structured meals per day — and raised a concern about loss of lean mass.
Improvement in some cardiometabolic markers. Some trials show improvements in blood pressure, triglycerides, and fasting glucose. These improvements often track with weight loss rather than being independent of it.
Simplicity, for some people. This is underrated and rarely measured. Some men find “I don’t eat before noon” much easier to follow than counting anything. Adherence is the variable that most reliably predicts results, and a rule you actually keep beats an optimal rule you abandon in three weeks.
What the Evidence Does Not Support
“Fasting resets your hormones.” There is no established scientific meaning to a hormonal “reset.” Fasting does change short-term hormone concentrations — insulin falls, growth hormone rises transiently, catecholamines shift — but these are normal, expected responses to not eating, not a repair process. Claims that fasting restores testosterone, fixes thyroid function, or corrects “hormonal imbalance” in men are not supported by good human trials. Some evidence actually points the other direction: a small trial in men doing resistance training (Moro et al.) reported lower testosterone with time-restricted eating.
“Your organs need a rest from digestion.” This is a metaphor, not physiology. Your liver, kidneys, pancreas, and gut are continuously active regardless of when you last ate; digestion is not a burden they recover from during a fasting window. When the digestive tract is genuinely rested for long periods, as in prolonged fasting or tube feeding withdrawal, the gut lining does not thrive — it atrophies.
“Fasting is superior to other approaches.” Head-to-head trials do not support superiority. The 2024 American Heart Association science advisory and multiple systematic reviews describe intermittent fasting as a reasonable option that produces results comparable to continuous calorie restriction, not better than it.
“Fasting is proven to extend human lifespan.” Lifespan extension from caloric restriction is well documented in some animal models. Human lifespan data does not exist and, realistically, may never exist in randomized form.
Safety: Who Should Be Cautious or Avoid It
This section matters more than the benefits section, especially after 40.
Intermittent fasting is generally not appropriate, or requires medical supervision, if you:
- Take insulin or sulfonylureas, or any medication where skipping meals risks hypoglycemia
- Take medications that must be taken with food, or on a fixed schedule
- Have a history of an eating disorder or disordered eating patterns
- Have type 1 diabetes, or type 2 diabetes managed with glucose-lowering drugs
- Are underweight, frail, or losing weight unintentionally
- Have advanced kidney or liver disease
- Are recovering from surgery, illness, or significant injury
- Have a history of fainting, low blood pressure, or gout flares triggered by fasting
Two age-specific concerns deserve emphasis for men over 40:
Muscle mass. Age-related loss of muscle is one of the most consequential things that happens to men in this window. Any weight-loss approach that compresses eating opportunities can make it harder to get enough protein and can accelerate lean mass loss. If you fast, resistance training and adequate protein are not optional extras.
Medication timing. Most men over 50 take at least one prescription medication. Changing when you eat changes when and how those medications are absorbed. That is a conversation with a pharmacist or physician, not a decision to make from a podcast.
Try This: A Low-Risk Way to Explore the Question
This is a self-experiment in observation, not a prescription. It is deliberately mild.

Start by describing, not changing. For one week, write down only the time of your first and last calorie each day. No changes. Most men are surprised — many are already eating within an 11- or 12-hour window and simply didn’t know.

Pick one small, specific change. For example: “I stop eating three hours before bed on weeknights.” Not “I’m doing 16:8 starting Monday.”

Name what you’re actually watching. Sleep quality? Afternoon energy? Evening grazing? Choose one or two things you can notice, and rate them 1–10 each evening. This keeps the experiment about your life rather than about a number on a scale.

Run it for two weeks, then decide. Ask three questions: Did it get easier or harder? What did it cost me socially? Would I want to still be doing this in six months?

Keep the basics in view. Federal activity and dietary guidance address strength training, protein, and hydration. A registered dietitian or qualified exercise professional is the right person for individualized specifics. Notice whether the experiment crowds out habits you already value; if it does, that is useful information.
If the honest answer after two weeks is “this made me miserable and I thought about food constantly,” that is a valid and useful result. Stop.
Talk With a Qualified Healthcare Professional
Before changing your eating schedule, talk with your physician, a registered dietitian, or your pharmacist if any of the following apply:
- You take any prescription medication, especially for diabetes or blood pressure
- You have a diagnosed chronic condition
- You experience dizziness, confusion, shakiness, or unusual fatigue when you skip meals
- You have any history of disordered eating
- You are considering a fast longer than 24 hours
A health and wellness coach can help you clarify what you want to try and stay consistent with a plan you chose. A coach cannot and should not diagnose a condition, interpret lab results, tell someone to change a medication, or prescribe a meal plan. Those belong to licensed clinicians. If someone selling a fasting program is willing to do those things, that is a reason to walk away.
Evidence Note
Reasonably established: Intermittent fasting can produce modest weight loss; it performs roughly on par with continuous calorie restriction in head-to-head trials; adherence, not the specific pattern, is the strongest predictor of results; hypoglycemia risk is real for people on certain diabetes medications.
Promising but limited: Independent effects on insulin sensitivity beyond weight loss; effects on blood pressure and lipids separate from weight change; benefits of early-day versus late-day eating windows. These come from small, short trials with mixed results.
Not established: “Hormonal reset,” organ “rest,” proven human lifespan extension, superiority over other calorie-reduction approaches, and claims about fixing testosterone or thyroid function.

Intermittent fasting is a tool. It is not the tool, and after 40 the question worth asking is not “does fasting work?” but “what eating pattern can I actually sustain for the next twenty years while keeping my muscle, my energy, and my social life intact?”
